City of Chicago Employee COVID-19 Testing Portal

Welcome to the City of Chicago COVID-19 Testing Portal. Any employee who is not fully vaccinated must undergo COVID-19 on a twice weekly basis with tests separated by 3-4 days. You must use this portal to report COVID-19 in compliance with the City of Chicago COVID-19 Vaccination Policy. You should report each test as soon as you receive your results.

Please note that for at home tests, you will need to provide a photo of your results that includes the unique bar code of the test you took, along with your name and the date you took the test written on the test card next to the unique bar code. If you fail to follow these instructions, your test results will not be considered valid.

If you test positive for COVID-19 you must immediately report that information to your department. You must not report to work if you have received a positive result. Your department’s HR Liaison will provide you with further direction on when you can return to work.

In accordance with the requirements of the Americans with Disabilities Act, the information related to testing collected through this Portal will be treated as a confidential medical record and will be retained by the Department of Human Resources in a separate confidential file. Such information will only be shared in accordance with the Americans with Disabilities Act.
