CPR Application

Program Overview

This program provides low-income motorists with relief for vehicle-related debt, excluding expired meter tickets. Benefits and requirements of CPR include:

Pay off the original fine amount for tickets issued in the last 3 years and all eligible older debt will be waived.
Applicants are required to have outstanding vehicle-related debt and be the registered owner of the vehicle associated with the debt.
To qualify, an applicant must be a member of a household currently enrolled in the Utility Billing Relief Program, the Administrative Debt Relief Program, or have a household income of less than 300% of the Federal Poverty Guidelines.

If you have multiple notice numbers or your driver’s license number is not linked to all of your tickets, you must call 312-744-7275 to consolidate your debt prior to enrollment.

Application Terms and Conditions

By entering my digital signature below, I certify that I understand and agree to the following terms and conditions:

I understand that in order to qualify for the Clear Path Relief Pilot Program (CPR), I must be the registered owner of the vehicle with associated debt, and I must either reside at an address that currently participates in the Utility Billing Relief (UBR), be currently enrolled in the Administrative Debt Relief (ADR) program, or have a household income of less than 300% of the Federal Poverty Guidelines.
If I move from the address that is enrolled in UBR or my household income changes while enrolled in CPR, I must notify the City of Chicago within 14 calendar days, and my eligibility for the program will be re-evaluated.
I understand that if I am enrolled in CPR, I must complete my 24-month debt relief payment plan before any eligible debt is waived.
I understand that if I am enrolled in CPR and have an active debt relief payment plan, I will be exempt from enforcement action (e.g. booting) on that debt.
I understand that if I default on my debt relief payment plan, I will have up to 90 days from the date of the missed payment to re-enroll in a plan and that if I do not come into compliance within that timeframe that I will not be allowed to re-enroll again and I will be eligible for enforcement action (e.g. booting) on my outstanding debt.
I understand if I default three times on my debt relief payment plan, I will not be allowed to re-enroll again and I will be eligible for enforcement action (e.g. booting) on my outstanding debt.
I understand that expired meter tickets are excluded from CPR benefits and must be paid in full as part of the debt relief payment plan.
I understand that I may only be granted debt relief for one application under CPR.
If this certification is being submitted by a preparer instead of the registered owner, the preparer of this application certifies that the registered owner understands the terms and conditions on this page and grants permission to the preparer to attest to these terms and conditions on their behalf.

All information submitted with my application for CPR is true and correct to the best of my knowledge. I understand that intentionally omitting a material fact relevant to my CPR application, providing a false statement of material fact in my CPR application, or materially altering documents submitted to show my income eligibility for CPR, may result in the denial of my application for CPR or my removal from CPR and the forfeiting of any CPR benefits, as appropriate. Such action may also prevent me from re-enrolling in CPR and may subject me to other applicable penalties as provided by law.
