City Vehicle Sticker & Daily Parking Passes Information Registration Form

The Office of the City Clerk Information Registration Form is used to gather all required documentation for the purchase of Vehicle Stickers & Daily Parking Passes. The Office of the City Clerk will review your submission and follow-up according to the information provided.

PLEASE NOTE: It may take up to 10 business days for our office to respond.

You must purchase a Chicago Vehicle Sticker within 30 days of residing in the City of Chicago or acquiring a new vehicle to avoid late fees and fines. Submitting your request within the 30-day grace period will not result in waiving of potential late fees and/or back charges.

To add OR change a Residential Permit Parking Zone to your City Sticker you must bring the following items to an Office of the City Clerk location.

  • Your current City Vehicle Sticker.
  • Valid Government Issued Photo ID (Driver's License/State ID).
  • Proof of address (lease, mortgage, utility bill within 30-days, or USPS Change of Address).
  • Adding or changing a Residential Permit Parking Zone cannot be done online, this request can only be completed at an Office of the City Clerk location.

A replacement sticker is for the exact same vehicle with the same VIN and plate as originally issued.

  • Please bring in the following documents:
    • The original City Sticker purchase receipt.
    • A police report (that specifies that the current vehicle sticker was lost or stolen).
    • A photo ID that matches the name on the City Sticker record.
  • If your vehicle's windshield was damaged or replaced, you must surrender your old City Sticker or provide a bill for the windshield replacement and pay a $5 replacement fee. A police report is not required. Replacements cannot be performed online.

A senior citizen is a Chicago resident who is 65 years old or older by the Sticker Term Start. In the case of new vehicles/residents, Term Start is the first day of the month in which you purchased the vehicle or moved to the City of Chicago. In the case of renewals, Term Start is the day after your current City Sticker expires. Seniors are eligible to receive one discount for a Passenger (P), Large Passenger (LP) or Motorbike (MB) vehicle and one discount for a Small Truck (ST). To qualify for a senior discount please:

  • Upload a copy of your State Vehicle Registration Card (including name and address).
  • Upload a copy of your Valid Government Issued Photo ID.

I am not longer a resident of the City of Chicago or own a vehicle that requires a Chicago Vehicle Sticker (City Sticker). All Chicago residents using his or her vehicle in the City must have a Chicago City Vehicle Sticker, regardless of where the vehicle is registered. Failure to do so can result in a minimum $200.00 ticket and additional fees. Tickets can be issued every day until vehicle is in compliance with the Wheel Tax.

If your license plate does not match your renewal form, please submit the following:

To update your vehicle’s sticker to reflect the newly issued license plate, please visit a City Clerk location in person and bring the following:

  • Your current City Vehicle Sticker
  • New full State Vehicle Registration Card
  • Valid Government Issued Photo ID

Updating your City Sticker’s license plate cannot be done online, plate updates can only be completed at an Office of the City Clerk location. Plate updates are free of charge.

If you purchased a new vehicle and need to transfer your current City Sticker to your new Vehicle, you must bring the following items to an Office of the City Clerk location:

  • Your current City Vehicle Sticker OR a total loss letter from your insurance company
  • Valid Government Issued Photo ID
  • New State Vehicle Registration Card

Transfers cannot be done online, transfers can only be completed at an Office of the City Clerk location

If your VIN does not match your renewal notice, you'll need to renew your old City Sticker and transfer that sticker to the new vehicle in order to avoid potentially paying late fees and back charges. To renew and transfer your City Sticker, you must bring the following items to an Office of the City Clerk location:

  • Your renewal notice, or the old City Sticker from the previous vehicle
  • Valid Government Issued Photo ID
  • New State Vehicle Registration Card

Renew and transfers cannot be done online, transfers can only be completed at an Office of the City Clerk location
