Benefits Questions or Inquiries

  • Contact the Benefits Service Center (BSC) at 1-877-299-5111 for immediate assistance. The BSC call center hours of operation are Monday through Friday (except holidays), from 8:00 am. to 5:00 p.m. CT.

  • You may enroll, make changes to your benefits, and submit dependent eligibility documents at www.cityofchicagobenefits.org.

  • You may send dependent eligibility documents via secure fax to 1-412-235-6797 or mail your documents to: City of Chicago, Benefits Service Center, P.O. Box 534077, St. Petersburg, Florida 33747-4077.

    You must submit government-issued certified documents (the Chicago Benefits Office reserves the right to request the original documents). If the certified eligibility documents are in a language other than English, you must have the documents translated and certified by the appropriate Consulate General.

If you need to speak to a Department of Finance – Chicago Benefits Office representative, please complete the form below. You will initially receive a call or email (based on your preference) from a representative. If necessary, an in-person meeting may be scheduled at that time.

If you are considering emailing personal information to the Chicago Benefits Office, please be aware that email and other electronic communication can be intercepted in transmission or misdirected. Therefore, please consider communicating sensitive information to the Chicago Benefits Office by telephone, secured fax, or regular mail.
